Á La Carte HTMA

Please print out the HTMA Requisite Form, fill out Part 2, and then send your hair sample inside a paper envelope.

HTMA collection instructions are located here. Do not tape the hair sample or put it into a plastic bag. It can go straight into the envelope with your requisite form.

Please complete your intake form.

The HTMA results take between 2 and 4 weeks. When I receive them, I will complete your analysis and custom protocol. You will be sent a link to your google drive which will house the lab results, a written analysis, video interpretation and protocol. The email will be coming from lacy@lacedinwellness.com.
